Oak Street Health is a network of primary care practices for adults on Medicare. Our approach is well care, not sick care. By keeping thousands of patients happy, healthy and out of the hospital, we’ve built a healthcare model that works.

Personal
Nothing is more personal than your health. Our care is personal, often spending more than twice as long or longer with our patients and knowing them as individuals.

Equitable
You should have access to the same great care no matter where you live. We invest in our communities by building Oak Street Health centers where healthcare services are needed most.

Accountable
Many adults on Medicare have chronic and complex conditions. We hold ourselves accountable for our patients’ health by taking on the risk and cost of their care.

Everyone deserves great healthcare.

Older adults have specific healthcare needs. Many don’t receive it. When they do, appointments are brief, in-network doctors can be far away and they don’t get personalized treatment plans. We’re changing all of that.

We’re all in on helping patients live healthier.

Most primary care practices follow fee-for-service models, which often prioritize volume of patient visits over quality of care. Oak Street Health operates under a value-based managed care model, meaning we take on the risk of our patients’ health?—?including primary, specialty, acute and post-acute care. Our model incentivizes high-quality healthcare because patient outcomes are tied to our success.
